    Leading home care agency is looking to hire a Home Care Liaison. We are looking for entrepreneurial, self-motivated individuals who excel at creating & building relationships, enjoy designing and implementing programs and working in a team environment. Our ideal candidate has prior experience as a healthcare professional (Discharge Planner, Social Worker, RN), specifically experience and exposure within the home care, home health, assisted/skilled facilities, hospital and/or care management industries. Additionally, there are direct client-interacting responsibilities as well so we will need an exceptionally caring person who is able to speak incredibly well with clients, educating them about our services and managing their care needs as well. This position will report to the Vice President of Business Development.

    Primary responsibilities include: 
    - Calling on and developing new referral accounts in the local healthcare and senior care community with elder care industry professionals, organizations and establishments
    - Coordinating post discharge home care with hospital and skilled nursing facility discharge planners, RNs, social workers and physicians to provide a seamless transition and effective rehabilitation at home for our clients
    - Representing our services in a professional and creative manner
    - Answering and actively making calls to prospective clients in need of care/services
    - Working directly with new & existing clients to make sure all plans of care are being met
    - Implementing and leading in-services
    - Organizing Lunch and Learn programs for professionals involved with the senior community
    - Representing the company at fundraising and support events
    - Working closely with the client service team to ensure client satisfaction
    - Maintaining accurate records on all prospective and active referral sources
